Vetting Forms
Organisations are required to edit the form to include their name and individual agency code. The agency code is a unique identifier that confirms authority to use the vetting service. No other alterations to the form are permitted.
Vetting can only be carried out with the consent of the applicant provided on the correct form. Potential employees should be told they are to be vetted and offered an explanation of the process (including direction to the Police website).
It is the organisation's responsibility to ensure that the Consent to Disclosure form is completed correctly. It is essential that all known names are listed (real, maiden, and other surnames). The identity and date of birth of each applicant must be verified against a driver's licence or passport.
Please assess the role your applicant will be acting in, and choose from the three options below which form would be most appropriate. It is important to select the correct form in order to avoid a breach of the Criminal Records (Clean Slate) Act.
Section 7
Use the Section 7 form if your staff are NOT acting in a role in which they are the main caregiver for a child, unsupervised and one-on-one in the child's or caregiver’s home. (i.e. Exception 19(3) (e) of the Criminal Records (Clean Slate) Act 2004 does not apply.)
Examples of such roles include: School Volunteers, School Contractors (working within school business hours only), After School Childcare Centre or Holiday Programme Staff, Rest Home Staff, Teachers, Teachers Aides, and Sunday School Teachers.

Section 19
Use the Section 19 (Exception) form when the role of the person you are vetting mainly involves care-giving rather than education of a child or young person. (i.e. Exception 19(3) (e) in the Criminal Records (Clean Slate) Act 2004 applies.)
'Exception' forms apply when the eligible individual has made an application to act in a role predominantly involving the care and protection of, but not predominantly involving the delivery of education to, a child or young person. Police understand this to mean that the person meets this exception when they are responsible for the day to day, and/or overnight, care of a child or young person.
Examples of such roles include: Foster Parent, Homestay Parent (inclusive of family members), Caregiver (other family members will complete Section 7 form), Nanny, Babysitter, Mentors, registered Medical Staff, and Social Workers.

Section 14
Vetting requests where the subjects are applying for a role outside New Zealand. Under section 14(b)(ii) of the Criminal Records (Clean Slate) Act 2004, the subject's full criminal history will be disclosed.

Please do not ask your applicants to fill more than one form. It is an either/or decision depending on the role the person will be acting in.
