When a requester makes a Privacy Act 2020 request to Police on behalf of another individual, Police must ensure that the requester has the written authority of the individual to obtain their personal information.

This form gives a person you nominate authority to request and receive your personal information from New Zealand Police.

When an individual makes a request to Police for their personal information under the Privacy Act 2020, Police are required to be satisfied as to the requester’s identify before providing access to the information.

If you are able to submit to Police a clear and readable photograph of yourself holding your evidence of identity, an identity referee’s statement is not required.
