1. Mental Health Services
Request for Non-Emergency Police assistance

This form should be completed by Mental Health Services requesting Non-Emergency Police assistance.

These may be requests under legislation (including transport) and attendance at Mental Health facilities/ inpatient units.

Police should only be requested when Police powers or capabilities are required.

2. Mental Health Services/Emergency Department
Missing Person report

This form should be completed by Health New Zealand Mental Health Services and Emergency Departments to notify Police when:

1. People go missing from, or are under the care of, Mental Health Services

2. People who go missing from ED after presenting for/with Mental Health concerns.

These reports must be based on clinical risk and/or legislation requirements.
