State Highway 5 has reopened following the fatal crash at Waimangu, south of Rotorua.
Investigations have now established that three vehicles were involved in the crash, which happened at about 4.45pm on Sunday 21 August 2016.
One person has died, two are in a critical condition and one is seriously injured. Three others have suffered minor to moderate injuries.
The serious crash unit is investigating.
