If you have concerns about your safety, or the safety of someone you care about, in a current (new or established) relationship, please complete a form to request information under the Family Violence Information Disclosure Scheme (FVIDS).
What is the Family Violence Information Disclosure Scheme (FVIDS) ?
The Family Violence Information Disclosure Scheme enables Police to consider sharing information about a person’s history of family violence where it is necessary to help prevent or reduce the risk of harm.
The purpose of the scheme is to help people make informed decisions about their safety, their relationships, and the wellbeing of their whānau.
Applications can be made by:
- A person who has concerns about their own safety
- A family member, friend, or other concerned person
- Police in situations where there are concerns for a person’s safety
Police will assess every application individually and consider whether the legal threshold for disclosure has been met.
When might you consider making an application?
You may wish to make an application if you have concerns that your partner has a history of family violence or behaviours that place you or someone else at risk.
Examples may include:
- Physical violence
- Sexual violence
- Threats to harm or kill
- Coercive or controlling behaviour
- Stalking or intimidation
- Psychological or emotional abuse
- Violence towards children, whānau members, or animals
- Patterns of behaviour that cause fear for personal safety
Police must carefully balance privacy obligations with the need to prevent harm and keep people safe. Submitting an application does not mean you are guaranteed that information will be disclosed.
Is it an emergency?
Is there an immediate, imminent, or ongoing risk:
- To life or property
- Of a serious offence or incident occurring; or
- A serious offence or incident is occurring now or has occurred within the last 24 hours
If so, please call 111 immediately.
If it is not an emergency?
If the situation does not require an urgent police or emergency services response and no one is in immediate danger, click the button below to make an application.
Please note:
If there is any risk in completing this form because the person causing harm may see what you are doing, leave this page immediately and access it again when it is safe to do so.
